This female patient underwent revision rhinoplasty for a number of concerns. On her frontal view, she was seen to have left midvault collapse, a pinched tip and nasal tip deviation towards the left. On the profile view, there was dorsal disproportion with dorsal excess in the supratip area and alar retraction. Functionally she experienced bilateral nasal obstruction. Correction was accomplished through an open rhinoplasty approach. The patient underwent septoplasty, placement of bilateral extended spreader grafts and placement of a septal caudal extension graft.
