This patient underwent rhinoplasty for concerns which included her nasal width and tip ptosis. Correction was accomplished through an open rhinoplasty approach. The patient underwent modest dorsal reduction and medial, intermediate and lateral osteotomies. The midvault was reconstituted with autospreader flaps. A septal caudal extension graft was placed and a lateral crural steal maneuver was performed to enhance projection. Bilateral articular contour grafts were placed. Cartilage graft material was harvested from her ear.
