This female patient underwent corrective rhinoplasty for a number of concerns after trauma. On her frontal view, she was seen to have left midvault collapse from a left upper lateral cartilage avulsion. She had septal and nasal tip deviation towards the right. She had a previous septoplasty. On the profile view, there was a moderate dorsal convexity. Functionally she experienced left nasal obstruction from a septal deviation. Correction was accomplished through an open rhinoplasty approach. The patient underwent septoplasty, medial osteotomies, placement of bilateral extended spreader grafts and placement of a columellar strut graft. A cap graft was placed at the tip. Cartilage graft source was an ear harvest site.
