This male patient sought consultation because of a septal fracture, left sided nasal obstruction and overprojection of his nose. Surgical correction was accomplished by performing nasal septoplasty, dorsal reduction, and reconstitution of the midvault with autospreader flaps. Tip rotation and deprojection was accomplished by septal shortening and medial and lateral crural truncation. The tip was reconstituted with intra and interdomal sutures and the placement of a cap graft. The septum provided adequate cartilage for the cap graft. The procedure was performed via an open approach.
